FAMILIES with CHILDREN in Public, Charter, Private, Parochial, Preschool - DC Health recognizes the importance of
vaccinations for preventing disease and reducing the dangers that can come with being exposed to certain diseases. This
document outlines the vaccines requirements based on age for all students upon enrollment in schools, reflecting recent
changes to the CDC Child and Adolescent Immunization Schedule 2022.
All students attending school in the District of Columbia must present proof of appropriately spaced immunizations
annually, by the first day of school.
Please complete and return your student’s school health forms including the Universal Health Certificate and Oral
Health Assessment Form.
